Understanding Chronic Neck Pain

Chronic neck pain can arise from various structural and degenerative conditions. Common causes include:

  • Cervical degenerative disc disease – progressive wear of the intervertebral discs.
  • Cervical disc herniation – protrusion of disc material causing nerve irritation.
  • Cervical radiculopathy – nerve root compression leading to radiating arm pain or weakness.
  • Facet joint arthritis – degeneration of the small joints connecting vertebrae.
  • Spinal stenosis (cervical) – narrowing of the spinal canal, sometimes causing neurological symptoms.
  • Whiplash injury – trauma-induced cervical ligament or disc injury.
  • Bulging disc – annular protrusion that may or may not impinge nerves.

Each patient’s presentation is unique, which is why personalized assessment is critical.

Regenerative Medicine Therapy: Non-Surgical Options

Regenerative medicine therapy uses autologous biologics to promote tissue repair and reduce inflammation. These therapies aim to restore cervical function and relieve pain without surgery. Common biologic options include:

Stem Cell Injection Therapy

  • Mesenchymal stem cells (MSCs) – multipotent regenerative cells capable of differentiating into bone, cartilage, or connective tissue.
  • Adipose-derived stem cells – collected from fat tissue via an adipose tissue harvesting procedure.
  • Bone marrow aspirate concentrate (BMAC) – collected through a bone marrow aspiration procedure, concentrating regenerative cells.
  • Image-guided spine injection or ultrasound-guided injection ensures precise delivery to the affected cervical region.
  • Fluoroscopy guidance may also be used for real-time visualization.

These injections are typically same-day autologous use to comply with FDA regulations on biologics and maintain sterile processing protocols. Informed consent for biologics is obtained to discuss potential risks, limitations, and uncertainties.

Platelet-Rich Plasma (PRP) Injection Therapy

  • PRP injection therapy leverages concentrated growth factors and cytokine signaling proteins to promote healing.
  • Blood is processed using a centrifuge processing system, and the injection is delivered under ultrasound-guided or fluoroscopy guidance.

IV Regenerative Therapy and Stem Cell Infusions

  • In select cases, IV stem cell infusion or IV regenerative therapy may be considered for systemic delivery of autologous biologics.
  • These treatments are generally reserved for patients with diffuse musculoskeletal inflammation or multi-joint involvement.

Who Is a Candidate?

Ideal candidates for regenerative therapies include patients who:

  • Have persistent cervical discomfort despite conservative care such as physical therapy, medications, or activity modification.
  • Are motivated to pursue non-surgical neck pain treatment.
  • Are medically cleared for minimally invasive procedures.

Patients with active infection, uncontrolled systemic disease, or severe spinal cord compression may not be appropriate candidates and might benefit from alternative interventions, including surgical evaluation.

I saw Dr Al-Rashid for a painful hip in 2023. I was at a point where I had to use my hands to “lift” my leg into the car because the pain was too intense to just lift it on its own. He said that I was a candidate for PRP (platelet rich plasma) injections because I still have cartilage. Although they are not (yet) covered by insurance, it was an investment in my comfort that I am very happy with. It has been over a year, I have no pain whatsoever and can carry out my normal routine, and increased my exercise. I highly recommend Dr Al-Rashid and the Atlantis Orthopedic Group in Palm Beach Gardens.

Always outstanding! He did my wife’s hip a little over a year ago and the follow up was great. I need two knee replacements and went to him to look at them. I will start with the first one end of this month. The staff follow through as been excellent. The best news is he does surgery at Palms West Hospital.

I had my right hip replaced by Dr. Rashid and he was excellent. The operation went smoothly. I didn't even feel the prosthetic when I woke up from surgery.

Before all of this he was very careful and helpful, explaining everything in detail from beginning to end, even tried a few treatments before jumping into an operation.

(injections/pills/scans etc)

He explained the pros and con very well. His staff is excellent in the Loxahatchee and Lake Worth office. I'm very happy I found him. My leg feels superior than my normal leg and it's only been three weeks. The incision was tiny, and after seeing him three weeks after the operation (today), the prosthetic is perfectly in place. I barely have any pain other than wound aches from the muscle incision, which of course is healing every single day.

I no longer feel any electrical surges on my leg, spasms, or the leg not waking up when I'm in a sitting position or sleeping. Literally feels perfect. I have nothing negative to say, would definitely recommend him to everybody. I like the fact that he was very detailed with everything from beginning to end.

He takes his job very seriously well being relatable and is not the type of doctor that jumps into an instant operation, unless absolutely needed, which is positive.

Comparison to Surgical Options

While surgery can provide definitive structural correction (e.g., discectomy, spinal fusion), it carries higher risks, longer recovery, and potential complications such as adjacent segment disease. Regenerative medicine therapy offers a conservative, biologic alternative, particularly suited for patients seeking non-surgical approaches.

Frequently Asked Questions

What is the difference between BMAC and adipose-derived stem cells?

BMAC comes from the patient’s bone marrow, concentrating MSCs, while adipose-derived stem cells come from fat tissue. Both provide regenerative potential but differ in cell type and harvest procedure.

How many stem cell injections are usually needed?

Protocols vary by patient condition. Some receive a single stem cell injection therapy, while others may require multiple treatments based on response.

Is regenerative therapy safe?

When performed by a board-certified physician following sterile processing protocols and FDA-compliant autologous use, regenerative therapy is generally safe. However, all procedures carry inherent risks.

Will this therapy replace surgery?

Regenerative therapy is not a guaranteed replacement for surgery. Patients with severe structural compromise may still require surgical intervention.

How long before I notice improvement?

Some patients notice changes within weeks, but tissue regeneration can take months. Individual outcomes vary.
